1003 Added code to make the BLASTER env variable more complete. This fixes Quake
and probably some other games running in a DOS box. Also modified the INF files
so they have no references to Creative, only work with PNPB003 and CRX0002 and
there are two INF's now. One to be used with Compaq's DRVINS.EXE program and
the other to be used on all other devices.

1004 Added code to enable MUTE for the MIDI slider control and added a fix for the
 			Quake 2 audio problem.
			Files Changed: controls.c, init.c, mgxsb16.rc, mix_objs.c, mixer.h, cnfgmgr.h

1005 Fixed a bug that caused the midi output to mute when the user muted the wave output.
			Files Changed: controls.c

1006 Fixed the Quake I bug #1796 which was two problems. The mixer was hozing the
HW instance allocation in the system VM. Eggplant was also reseting the SB HW
which turned the CD volume off.
			Files Changed: mgxsb16.asm

1006xp Removed plug and play dependency from gxaudio.inf so driver can load with XpressROM.

1006xpb Removed MIDI functionality, since XpressROM does not enable MIDI.
