Form Fears

Flo: Forms! Forms! Forms! I hate filling out forms!

Phil: Everybody does.

Flo: I suppose you'll tell me they're necessary evils?

Phil: Yeh.

Phil is right at least about job applications being necessary. Few companies will hire without one. Job applications may request personal information about the applicant:

Name, address, telephone number, social security number.
Education
Work history
Special skills, training, knowledge
Other experience (clubs, volunteer work, etc.)
References

Filling out job applications will not make the Guinness Book of Favorite Pastimes. Before working with the parts of a typical application, read some tips to ease "application anxiety."

Attitude - Decide your application is your "calling card." If the job is worth having, the application is worth taking time to complete honestly and accurately. Don't try to do it on the bus or while you watch a soap opera.

A Keeper - Yes. Keep a copy. Refer to it when you fill out subsequent applications. Some parts will not change.

Neatness - Yes! Neatness counts. Ask for two applications (one for practice and to keep for your records, and one to turn in). Type or write legibly. If you write, use ink, not pencil. If your handwriting looks like a strange, new alphabet that nobody can read, try printing. If your printing is as poor as your handwriting, get someone to type or print your application for you. But be sure you sign it. Your signature is your handwritten legal name.

Personal Information

Name:

Use your legal name and middle initial:

Thomas A. Jones

or, if in small print the applications says "last name first":

Jones, Thomas A.

Type your legal name below, "last name first."

Address:

1114 Lilac Street
Ozark, Arkansas 72949

Include the apartment number, if you live in an apartment. Include your zip code.

Type your address below.

< >

Telephone:

List a telephone number where you can be reached. If no one is home to take calls during the day, include the number of someone who will take messages for you.

Type your telephone number and the number of someone who will take messages for you below.

< >

Social Security Number:

Very important! If you don't have a social security number, get one. Local telephone directories, libraries, or banks have the address of the social security office for your locality. Print your number as it is divided: 123 45 6789. You will need a social security number for the rest of your life. Once you get one, memorize the number. You can also write it down in several safe places. Protect the social security card. You don't need it at the beach!

For security purposes, you don't need to type your social security number here, but you can type a fake one if you want.

< >

Education:

Include the name of your grammar school and your high school. The form may say "degree." Write "diploma" or "GED" if you have one.

If you have received training from a vocational-technical or trade school or any other institution, be sure to include it, along with the course of study and the number of hours or credits you earned.

Type your educational history below.

Work History:

Do you think you don't have any? Maybe, you do! Think... have you ever earned money for babysitting, yard work, delivery of newspapers, or feeding the neighbors' pets while they went on vacation? The key here is work you did over a period of time that you earned money for, even if it only lasted a few weeks.

Don't undervalue any work experience. If you have any at all, list it. Let a potential employer be the judge of how it might apply to other kinds of work.

Follow the directions. Many applications will have you list your last job first and work backwards. Include the time period. Here is an example:

June 86 to Aug. 86
Jerry's Supermarket -- Carry-out worker
Jerry Niquist
1 Ozark Mall
Ozark, Arkansas 72949

Feb. 86 to May 86
Lin's Cafe -- Dishwasher
10 Ozark Mall
Ozark, Arkansas 72949

Type in your work experience below. Use the form used in the examples above.

Special Skills, Training, Knowledge

Here is your chance to list miscellaneous experience such as:

-- Licenses (driver's license)

-- Machines (typing, word processing, adding machine, computer, tractor)

-- Abilities (programming, data entry, Morse code, karate, basketball team, ballet, swim team, track, sewing, piano, guitar)

-- Certificates (water safety, gun safety, dog obedience training, first aid)

-- Other experience: (clubs, volunteer work, Girl Scouts, Boy Scouts, Junior Achievement, Ozark Children's Home Volunteer, 4-H Club, Future Homemakers Club, Y.W.C.A., Y.M.C.A.)

List your special skills, training, and knowledge below.

References:

Give some thought to whom you will list for references. Blood relatives are not usually accepted. Here are some ideas: neighbors, teachers, clergy, scout leaders, lifelong friends of your family who can vouch for your character.

A "good" reference is not necessarily the most famous person in town. List someone whom you feel will evaluate you fairly and enthusiastically in writing or by telephone. Be sure to include contact information: telephone number and address.

Type a list of three references below.
