The following document provides details of QLogic's PCI Fibre Channel StorMiniport driver 
changes between version 9.1.1.15 and 9.1.2.15.

    *   Add support to use combined 23xxipx firmware
    *   RISC code 3.03.19 (23xx)
    *   RISC code 4.00.17 (24xx)
    *   Add support for new STT flash chip SST25VF080B
    *   Add option for not to dump code section for 24XX firmware dump
    *   Added support for the following devices:

    *   Close Er44270: Serial number for the 4 G hba should be obtained from the VPD
    *   Close ER38787: W2K/qla2340/veritas tape backup long cable pull failed error mesage ERROR_DEVICE_NOT_CONNECTED.

    *   Close PTR 90652: Qlogic Driver returns a selection timeout when rebooting a controller in an OEM special dual loop configuration.

    *   Embed 24XX FW in the driver
    *   Close ER43175: New driver could not detect any device to install as BOOT from SAN on 
        one specific setup.  Investigation showed that driver detects the target device successfully but 
        failed the logins because the target had previously been logged in through by the HBA firmware.  
        Driver is changed to handle this particular case correctly.
